When starting a
Kingdoms campaign, you may select from a variety of difficulty options and game rules. A Custom difficulty is also available, which allows you to manually configure all options.

- Day Limit (41-99): If the enemy boss is not defeated before this day, the Kingdom will be overrun. (The game is lost.)
- Escalation 2 Day (5-19): On this day, Escalation advances from 1 to 2, which increases the danger in the Kingdom.
- Escalation 3 Day (20-40): On this day, Escalation advances from 2 to 3, which increases the danger in the Kingdom.
- Sieges (Normal, Increased): This setting influences how many sieges spawn to attack Inns.
- Heroes In Kingdom (4-13): This setting determines how many heroes are in the kingdom.
- Starting Skills: This setting determines what skills brand new heroes have.
- Standard: The heroes start with their core five skills.
- Mystery: The heroes start with a non-standard set of five skills. These skill packs are curated to ensure that they are useful. Choosing this option provides variety and challenge over the Standard starting skills.
- True Random: The heroes start with five truly random skills. This creates additional challenge.
- Hero Respawns: This setting determines what happens when heroes die.
- Permadeath: When a hero dies, there are no replacements.
- Respawn: When a hero dies, that same hero respawns but some mastery and upgrades are lost.
- Note: The Stagecoach must return to the location where the hero died in order for them to respawn. A respawning hero will gain a
Disease, lose two to four skill upgrades, and re-lock one to two skills outside of their starting five.

- New Hero: When a hero dies, a new hero spawns randomly in the Kingdom.
- Inn Loss Limit (1-100% of total inn count, rounded up): If this number of Inns are destroyed, the game is lost.
- Starting Paths:
- Wanderer: The heroes start with the Wanderer path.
- Random: The heroes start with a random path.
- Full Selection: Hero paths can be selected at The Crossroads.
Escalation
Escalation is a Kingdom-wide mechanic that increases the difficulty of campaign over time. The campaign begins at Escalation 1, and there are two Escalation Surge! events that increase the Escalation level and spawn extra sieges. Certain quest objectives, events, and encounters can only occur at specific levels.
